JOIN THE REGISTRY

How to Register
Joining the Registry is easy and free.
Anyone 50 years old or older can enroll.
You can do it without ever leaving home.
Just call us at 602.839.6500 or send us an
E-mail at registryinfo@azalz.org to let us
know that you would like more information
about the project. We will contact you and answer any questions you may have.

We will send the registration forms which include a detailed questionnaire about your medical and family history and a consent
form for you to complete and return via fax
or U.S. postal mail.

When we receive your completed forms,
we will contact you again. We will review the completed questionnaire which includes questions about the studies you would like
to participate in. We will also ask you some additional questions about your thinking, memory and daily functioning.

Once your registration is complete, we will refer you to the types of Alzheimer's related clinical research studies you are interested in at the locations you have specified.

Participation in any study is completely voluntary. Any medication or treatment being studied is provided free of charge. Examinations, laboratory tests and other procedures done during the study are also free.

Your personal information is protected in accordance with federal HIPAA guidelines unless you provide written consent stating otherwise. You can leave the Registry at any time.
